Mendocino Care Network (MCN)—a branch of ResolutionCare—is seeking a dynamic, creative and professional part-time BSW/MSW to join their interdisciplinary medical team. The ideal candidate has experience with palliative care/hospice/oncological social work and is comfortable providing home-based social work services through a combination of tele-health, telephone outreach, and home visits.

The ideal candidate is comfortable working independently and as part of the collaborative team, has strong communication skills, and can maintain healthy and professional boundaries. Candidate must be comfortable with documentation on an EMR and have computer skills. Multi-lingual skills are highly valued.

The position is initially for 20 hours/week with possible future progression to FT status as the patient census grows. Additional training and supervision in palliative care services will be provided as appropriate. This position is based in our central Mendocino County office and serves patients throughout the county and beyond.
